First, let's include only guidelines that were directly addressed by the trial.

Second, let's have conventions for the Guidelines section. I've noticed alot of variability in how these are described. In order to bring uniformity and a more professional balance, I suggest the following format. Feel free to make suggestions or changes.

For example:

• ICD therapy is indicated in patients with LVEF≤35% due to prior MI who are at least 40 days post-MI and are in NYHA functional Class II or III (ACC/AHA/HRS; IA)
• ICD therapy is indicated in patients with LV dysfunction due to prior MI who are ≥40 days post-MI, have an LVEF≤30%, and are in NYHA functional Class I (ACC/AHA/HRS; IA).
